In this session, we will evaluate your medications’ effectiveness, address any potential side effects, and explore lifestyle modifications. Any necessary adjustments to the treatment plan will be made based on our discussion.

Transfer Patient | 45 Minutes
*For medication and continuation of care, transfer patients
*Appointment can be in-person or Telehealth
This session is exclusively for established transfer patients seeking medication management and ongoing care continuation. Only patients transferring from other providers will be accommodated during this appointment.

+ ESA letters, school, or work accommodation letters for ADHD
+ Phone calls greater than 15 minutes

*As a practice we do not complete short term disability paperwork or conduct court ordered evaluations.
Appointments cancelled or rescheduled need a 24-hour notice prior to start time of appointment to avoid a no-show fee, which is half of the self-pay cost of the designated appointment.
Please note that insurance does not cover no show fees and it will be charged to the credit card on file.
